What to Bring
Each person must present a valid photo ID at the time of fingerprinting. Revoked, suspended or expired ID will not be accepted.
Examples of acceptable ID:
- (Any) State Driver’s License
- (Any) State ID
- DMV Driver’s Permit
- US Passport
- Permanent Resident Card
- Military ID
ONLY IF, you are obtaining fingerprints for the Department of Health and Human Services (DHHS) will you be required to provide a fingerprint authorization form from your employer/company. This form will include a 6-digit number specifically assigned to you that is required for processing.
